Deposit, Cancellation & Re-Schedule Policy
• A deposit is required to book an appointment.
• All deposits are non-refundable and non-transferable.
Re-Scheduling
• If you need to re-schedule, a 72-hour notice is required to keep your deposit and apply it to a new date.
• A maximum of two (2) re-schedules are allowed.
• If you re-schedule more than twice, a new deposit will be required to book another appointment.
Cancellation
• Cancellations made with less than 72 hours notice will result in the loss of your deposit.
Agreement
• By submitting this form, you confirm that you have read and agreed to the Deposit, Cancellation, and Re-Schedule Policy.
